NS&I Premium Bonds Application Form Question

Hi everyone,
My mum wants to open a NS&I Premium Bond for my younger sibling (I know these arent the best savings bet) but since she is opening it on his behalf, and she has a few queries about the application form, which haven't been made clear on the form or in the brochure.
On the form, that has arrived by post, we have 5 sections,
This is the first bond we will have with NS&I hence we don't have a holders number, so we will leave Section 2 blank.
In section 3, we will include my young brothers details, because he will be the holder, (it says above the form, that Buying for a child under 16: Give their name, address and DOB and the parents or legally appointed guardians details).
In section 4a, we will enter my Mums details.
But here is where the question lies, Who signs section 5, which says the applicants signature. It says that if you have read the T&Cs, sign this declaration, but who?? Is it my mum or my brother?
