Only an application form gets reference details from me! avoid it really anywhere else cause I don't want the worry of ex employers being contacted willy nilly though I don't suppose you can stop it? must also get people's backs up to receive reference request after reference request, though slightly different when I worked in care which was the only industry worked in, where references would be taken up before I even had the interview, though I imagine a CV/application was all the auth needed

That why it's 'on request' and probably not listing the names of people to contact etc
