You can apply to First Direct online, but as you're self employed, it may be a bit more involved. You might not get automatic acceptance, they will probably need some income details from you.0 -

I'm a self employed web developer so in terms of income, it's hard to state an exact amount for eligibility criteria. Some years I've made three times what I have in others. So, it's also easier if this can be avoided.
Ideally, I'd like to pay around £10-£12 a month and if possible, have mobile phone insurance included.
The N&P card seems nice, but there are no benefits whatsoever to paying a monthly fee.
Any help would be appreciated.
- I've never liked Halifax and I'm not sure why. Maybe it's because they appear to try too hard to attract customers and that puts me off but if anyone can prove otherwise, I'm willing to consider it as well as any recommendations!

Having read through your list of requirements I would say that Barclays would be the obvious choice. I have an Additions Active Account which I pay fifteen pounds per month for. This gives me:
Mobile phone insurance.
Worldwide travel insurance
ID theft assistance.
RAC comprehensive cover
24/7 legal helpline
Card registration to notify of theft or loss with one call, this includes lost luggage and key retrieval.
Free will writing service.
Gadget insurance.
This above costs £180 per year, if you were to buy the top level of RAC cover as a stand alone product that is about £140 so as you can see this account is a clear winner. As you are very particular about what your debit card looks like this account will suit you down to the ground as you can design your own debit card.
