ES10 form

I'm trying to find out what an ES10 form is, my brother is disabled and has been told he may be able to reclaim monies using this form but that it is only available for the next few weeks. I cannot find what it relates to. Please can anyone assist

No such form - I think you mean IS10 form which from the little info I can gather is something to do with Claiming the disability premiums on JSA/IS/ESA for income related benefits, but someone else will know more than I.

It could be an IS10 which is the form that needs to be filled in to claim the Severe Disability Premium in both Income Support and income related Employment and Support Allowance.

The form can be obtained by phoning the DWP.

Apparently there is an ESA10 form, I've read that it's for a Hardship Payment of ESA.

IS10 for severe disability premium, payable if you have middle or high rate "DLA Care" & you live alone and no one gets Carers Allowance to care for you.

ESA10 for ESA hardship when you are not being paid ESA or are on reduced (sanctioned) ESA.
