How to fix duplicate pages in Google Webmaster?

So Google Webmaster have long been complaining about duplicate meta descriptions and title tags. The pages Google complain about are either old pages that no longer exist on my server (and have been fixed with a 301 redirect) or alternatives to the URL... For instance, /example.html and /example would appear as having the errors.
Now I have tried to fix this with canconical tags to no avail, I have ensured there are no mentions in sitemap, tried to even block with robots.txt as well as having consistency with hyperlinks.. What the eff can I do!?
