Joint tenancy

Can anyone advise can i do it myself or how much does it cost to change my house from tenants in common to joint tenants and how to go about it - I understand I need to fill in form RX3 but don't understand the bit about supporting evidence any help would be much appreciated. The reason for the change is to secure my future as my partner is still married but legally owns my house along with being responsible for half the mortgage.
