Change of name - Nationwide

I opened up a Nationwide FlexAccount in 2009 in my birth name, then in 2010 I got married, changed my surname on the account and then after the divorce I didn't change it back.
I've seemed to have misplaced the dissolution certificate and would like to change my name back, I have a passport and a driving license, both of which are in my birth name. I also have NatWest bank statements (March 2013) which are also in my birth name.
Are these enough for them to change the name back?
Advice would be greatly appreciated!

Yes both Passport And Driving Licence will be ok, though go into branch and fill the form in it will be quicker and sent via there internal post system and save you postage too.Time is a path from the past to the future and back again. The present is the crossroads of both. :cool:0 -
