When is a PPC required to provide you with a POPLA code

Bee reading a number of sites and, of course, on here. There is one current point that isn't clear to me and it does make a difference to the POPLA appeal process and timescale.
When a motorist makes an appeal to a PPC and that appeal is rejected, is it
(i) a requirement of the POPLA / BPA process that the motorist has to be given a POPLA code with the rejection letter or
(ii) is it sufficient for the PPC to refer the motorist to the POPLA process, web site etc, thereby requiring the motorist to contact the PPC to demand a POPLA appeal code?
Clearly if it is scenario (i) and the PPC does NOT supply a POPLA code with the initial rejection, then waiting out the required time (35 days) for them to do so should mean an automatic POPLA win based upon failure to follow process.
As I said, I can't find the answer to this and, as I have a question to POPLA still unanswered despite their target time of 5 days for a response, thought it best to ask here.

With regret, my vote goes to (ii), '...the operator should be contacted to obtain one.'
How to appeal
An appeal form, containing your 10 digit verification code, should be sent by the operator with the rejection of your representations. If this form is not enclosed, the operator should be contacted to obtain one.

The BPA CoP states this, if they are providing a template letter it will have the code on it, if they are giving a link it should have the code, otherwise what would be the point of the link if you can't do anything?
The point is that on an appeal rejection the operator should give the details of popla, we know that some have no mention of this at all, or break the code of practice in other ways. A complaint has been submitted to the bpa, dvla & popla about a certain parking company by myself yesterday, we shall see22.12 If you reject a challenge you must:
• tell the driver how to make an appeal to POPLA.This includes providing a template ‘notice of appeal’ form, or a link to the appropriate website for lodging an appeal
• give the driver a reasonable amount of time to pay the charge before restarting the collection process. We recommend that you allow at least 35 days from the date you rejected the challenge.When posting a parking issue on MSE do not reveal any information that may enable PPCs to identify you. They DO monitor the forum.
We don't need the following to help you.
Name, Address, PCN Number, Exact Date Of Incident, Date On Invoice, Reg Number, Vehicle Picture, The Time You Entered & Left Car Park, Or The Amount of Time You Overstayed.
