Help understanding no claim discount

Hi, my Father has just asked me to renew his insurance but I'm confused regarding his no claims discount. He is currently with Aviva and had 5 years plus ncd however about a month ago his tyre burst and he damaged a park van. Details were given to Aviva but he heard nothing more, his renewal has just come which says 3 years ncd and his premium has gone up by over £100, I presume the accident has effected both. I wish to change insurers via Quidco for the cashback but am not sure what to put for his ncd. Any help appreciated asap as he needs to renew by tomorrow!!

(And a fault claim to disclose)0 -
As above......put whatever you can prove by means of written proof and also declare the accident.0
