    sorry my baby is due 3rd of august but im staring my maternity 2months early as on my previous pregnancy i had my baby 2months early. also im at uni so its getting to much for me.

    Despite having requested a number of times that you forward my SMP1 form i am yet to recieve it.

    Is it your intention to withhold this document?

    I require a response within 5 working days along with the required SMP form.

    Your response may be forwarded to ACAS should i be forced to seek further advice from them.

    Dont bother sending this letter to ACAS, but at least you can forward their response should they be awkward. I would hope it would jog them along.

    you could also add the dates you have requested this form and who you requested it from if you have the details, nothing worst than naming and shaming

    Your employer is obliged to provide you with a form SMP1 and the return of your original MATB1 form (if submitted) within 7 days.

    If you have had to wait longer than this your employers are not comlying with HMRC guidlines and should be reported as such. Just visit your local tax office, there's bound to be one closer than your work by the sounds of it and tell them about how difficult your employers are being. They aren't as scary as they sound and my local tax office is run by 3 friendly middle aged ladies who I'm sure would more than willingly call an employer and put the frighteners on them! and I'm sure a 'friendly' phonecall from the Revenue will be enough to kick their !!!!!! into gear.

    Seriously the excuses they are giving are unacceptable. SMP1's take seconds to complete, all it is, is ticking 2 or 3 boxes then a signature that's it.
