DVLA bans - which PPCs have been banned?

I raised this question in another thread, that has mysteriously vanished (or i cant find it)
Most people on here know about Combined parking solutions who were banned by the DVLA for misconduct ( bad signage?)
and of VCS ( or was it Excell)???
and the half hearted ban for TCP/Smart over poor signage, that has been magicaly rectified by a marker pen.
but who else has been on the naughty step, and more importatly what does it take to nominate a PPC to the DVLA's bad boys club?

thanks for that, i wasnt aware the Roxburghe were in the buisness of ( attmepting to) run car parks.From the Plain Language Commission:

Roxburghe was banned for saying that the RK was legally responsible for parking charges on their website pre October 2012
Excel Parking was banned for having signs that basically said the same as the above prior to October 2012. This was in Parc Tawe North in Swansea
