another excel question

Hi,
I want to make an electronic version of the McGill pain questionairre.

So first of all I want drop down boxes for the description words, is this possible?
Second, these words have a value to them in order to get the final score. If I use a drop down can I assign a value to the selected answer and if so get this totalled along with the other values?
If this is possible can anyone suggest relevent formulae?
Am i on a hiding to nothing and should I make it simpler by showing all the words and tick boxes (if possible) and using IF statements to provide a value for each ticked box?
Would it be easier to do this via database rather than spreadsheets?

Currently I complete a form every couple of weeks to keep a record of pain throughout the year alongside a diary (This changes to once a week for a month or so when I change meds). This means my Diary has lots of additional pages stapled in and it's a bit of a mess.
