Quick Advice Needed

I know this advice will be on the forum but wanted a quick answer.
I have a selection of accounts I want to pursue PPI refund on. I have the full accounbt number of some as I have the paperwork, and some I have the last 4 digits of the account number as appearing on my credit report.
Questions are:
1. For the accounts I have the full detail for, can I just send a letter off to ask the company to assess my eligibility for a refund (i.e. not using the templates set out on this forum).
2. Where I don't have the full account details, i guess I need to send a more formal letter requesting the paperwork with a covering cheque for £1?
3. How would I get the correct addresses? Some of my accounts are Virgin Money, which I believe are ran by MBNA - so which company / address should I write to?

1 - you can't ask for a refund, you do have to make a complaint that it was missold so use the template or phone and complain.
2 - if the accounts are closed you need the SAR and £10
