interest payments on instalments

what I don't understand is if you want to spread the cost of car insurance by paying monthly instalments you get stuck with such high APR. 23.4% on one of my policies.

By spreading the payments you are entering into a credit agreement and that is the cost that insurer charges (or its credit provider).
Bottom line premium is not the only cost to consider. Some insurers allow interest free installments.
