HSBC - Cardholder Repayment Protector

I'm in the middle of filling in the 'PPI Consumer Questionnaire' but just need a bit of advice of how to make sure its filled out correctly and making sure I'm making a solid claim.

Just having a bit of trouble filling out section E as I can't think what else to write and what I have wrote seems too 'short' - I've basically put where the PPI was taken out (in this case my branch of HSBC), mentioned they've been fined by the FSA and briefly mentioned that their salesperson was pushy (so I couldn't say no) and I wasn't informed that it was optional.
How can I expand on that?

Also, I do have a copy of my agreement so of course I'll be sending them a copy but I also came across something else - an 'Insurance sales acknowledgement slip'. This was never signed or dated so could this back my claim up too?

    You don't need to do this, just list your account numbers and details on the form.You don't need to send such documentation unless later prompted to.

    I think you'll find it was HSBC-owned HFC Bank who were fined.

    It's irrelevant to your complaint anyhow; it's whether you personally were mis-sold the PPI.

    Whoops, obviously I was wrong about HSBC.

    The form does ask for a policy number but I can't find one anywhere but I have provided by credit card account number.

    Are you certain you were paying PPI?

    It would have shown as a separate line on each of your monthly statements when the account had an outstanding balance.

    If it's not on your statements, then you don't have it...

    Yes I am certain. I've checked on my recent statement and its listed as 'insurance premium'.

    Also, I received an annual review last week which clearly states it is PPI and I also phoned HSBC to confirm this.

    HSBC should be able to give you the PPI policy number, if you can't find it on your statement or Annual Review.
