Repaying Rejected Crisis Loans

Hi All,
My Mum just received a letter from the DWP stating that she needs to start paying back the Crisis loan she had.
No other information just they are now going to be taking £12 each week.
The thing is, she has paid back £350 from a previous loan which she had received, this was from 5 years or so ago.She did have the letter that stated the loan had been repaid and the money would now go back to normal.Then she applied for a 2nd loan around 3 years ago which was rejected.We forgot all about it until I checked her statements and noticed it was short each week by £12, when she asked DWP, they claimed they had sent a Giro that was cashed and signed for and that she needs to pay.
She got in touch with CA whom took on the case, and asked for the signature and relevant information but never got a reply, this went on for 6 months or so, and finally the CA and ourselves just gave up, as there was zero communication from the DWP.
Now this morning, they are claiming that she needs to repay again for either the 2nd rejected loan, or a 3rd loan that she never applied for.
She did phone them and was told to put it in writing.
Now I am wondering where she stands in all of this, as DWP are demanding things but are not providing any information as to when the loan was applied for and paid out.And we are not clear if its for a 3rd loan or the 2nd that was rejected.
She has felt very angry/bitter about having to have paid back £350 for a loan she never had, and now they are doing it again.She's 70 and not really in the mood to have to take on the DWP again.
So is there anywhere or anything that we can do to get this dealt with?
I know details are a bit scetchy at the moment, but is there any advice as to where she stands and whom to get in touch with?
