Almost certainly the PIN on the card has been locked and all the OP has to do is go to a Lloyds ATM and unlock the PIN assuming he knows it.
Then just use the card in any other working card reader
Not a card - the device generates a 6 digit number when you press the button which is synchronised with some central computer

I also don't see why going into branch to solve the problem is such a problem especially as its only 5 miles away. I could understand if like me your branch is 15 miles away.. Take the ID in as requested and sort the problem out.
Try living in a rural area with just one bus every 1 1/2 hours or so - some of us don't have anything worthy of the name of public transport

" A payment coming in around the 28th ? Well if its in four figures it'll be from <my employer>. The other regular income I have is <e.g. around £80 a month child benefit from HMRC...>. Outgoings ? Well, I have a direct debit to pay the balance of my <XXX credit card> at the start of each month, and pay <utility supplier> a regular monthly payment of £xx around the <day of month>, and I last went to the ATM and withdrew <£xxx> on <day> "
They won't accept an "if then or else" answer, they just take the first one you give. If you honestly don't know it's an immediate fail and they can't give you a second chance.

    plusmore, raise a formal complaint.
Guess what - there is nowhere online to make a complaint not a phone number or email address!, I spoke to the call centre manager and asked her to make one - I doubt they will though.
