Voluntary Work and the Work Programme

I'm currently on the work programme and before Christmas I started voluntary work with Age UK. When I went into the job centre to sign on the person who took my form told me I would need to fill in a form. At that point a man started kicking off (throwing chairs, ripping telephones from the wall etc) and I was ushered out of the door by the security guard and JCP advisor, the JCP advisor told me to inform them next time I signed on.
The next time I signed on I explained what had happened and I was told that I have to fill in the form when I attend the work programme. The work programme provider (A4E) have told me that the I need to fill in the form at the job centre.
Does anyone happen to know which is these is correct? There seems to be a real lack of communication between the A4E and the Job Centre as its not the first time they've passed the buck to each other (different query last time!).
