I can't find the form but these guides give guidance on the completion of the form, and you can work out what the questions are from the information:

ive also got the original form but its with my GP practice.
A GP has lets say put a lot of rubbish on it and I want it changed, wont hold my breath with it though0 -

You can't ask a GP to alter an independent report! The report is compiled from the records held by the GP.
Are you suggesting that the GP has lied on the factual report form in that the contents of your medical file don't equate to what has been said?0
