Looking for form sa 3

Been told i need it to claim for dependants.

  • What benefit are you claiming? Posters will need to know that in order to tell you where to apply. I tried to google what you said but it comes up with self asessment forms on HMRC web site, so not sure that is what you needed.

    Was on incap, had the medcal and didn't get 15 points so appealed by letter and phoned today.The guy said i need a new doctors note and to get form SA 3.
    No letters through post hence me (trying) to remember what he said.
  • Yes you will probably need to go to your GP and get what is called a Fit note now (sick note before) - but I am not 100% certain of that as you are being migrated over from Incap ben. The other form is ESA3 and in that it does ask about children and their ages etc and if anyone in the home is disabled or gets DLA etc. They should post this form out to you. It is not available in PDF version.

    If you have only just sent in the appeal, then I expect that you will not have yet been awarded ESA full rate. That is what the Fit notes will be needed for. Have you asked for a copy of the ESA85 form that ATOS sends out to DWP, who make the decisions, so that you will know how you failed and on what basis. I think that there is also something called a statement of reasons and somebody else like Roger black willknow more.

    Yeah only recently sent it, received the letter today

    In order for us to consider payment of ESA whilst appeal is ongoing, provide fit note.
