MCOL di i put branches address as defendant?

Just doing finshing touches when filin on MCOL but where it says put defendants address do i put my local Halifax branch address???

Also one more thing i cantr seem to see this interest rate claiming box that every one else seems to see!!

Not sure what you mean. On the claim form in the 'particulars of claim' you will detail in there the interest % and total amount (this would be as per Martins advice thread and as you should have worked out using the interest calculator). The boxes which need to be filled out detailing amounts claimed are: amount claimed (including interest), court fee, solicitors costs (if applicable) and the total amount.
There is no separate box where you detail the interest - this should be detailed in your 'particulars of claim'.
