Telephone socket adapter that I can wire existing extension to?

Hi folks,,

A good number of years ago a sky guy did an extension from my bt phone socket to hook into my sky box, think he did a custom job i.e wired the cable up to the socket adapters at both ends of cable.

The adapter that hooks into the bt socket is now usless, probably wear and tear and the odd shoe kicking it by accident.

So rather than run a new extension lead I just want to wire up a new bt socket adpater, seems there are four little wires to wire up to an adapter, but the problem is where can I get an adpater somehwat similar to this that will allow me to easily wire in the existing cable.
