Cold bridging around windows reveals

There's alot of condensation forming on the plaster around our windows, which are uPVC. The wall, ie. window reveal, feels nuch colder in places and therefore causing condensation, which is why I think it is cold bridging.
I cannot see any obvious problems in how they were sealed inside or out, so am wondering if they were installed correctly, should any gaps between frame and wall be filled with expanding foam? Or is there another problem?
The house was built in 70s, has cavity wall and windows installed about 10 years ago, we only moved in 2010.
The problem is significantly worse on north facing walls.

The windows should be sealed with silicone, or similar, on both front and back faces. Are they, and on all four sides?
If there is a problem it is probably with the reveals. Are they dry lined or plastered? The window fitting may have damaged the reveals. In particular the cavity close detail.
Considering the age of the windows and the difficulty of work to the reveals there is little else I can suggest ... over to others.0
