Done.. but, boy, it's badly worded. Sorry to point this out but I'd have been shot [when I was at university] if I'd cobbled together something as slapdash as this.

I agree... I cannot see how any analyst would be able to get meaningful data from those questions... you really need to feed that back to your DD as feedback should always be welcomed.

Yeah one of the questions didn't make sense to me.
