Hi i cant findanyone to help me fill in the dla form thing i'm thinking of doing it online,

I ahve letters from the folwing doctors and other people but i dont know how to put it into the dla form

for my mental heath i have
Moderate level of anxity with modereate to serve level of depression and mod to serve on the psychological distress with panroia and avoidance disorder

awaiting counclling for depression

i have serve scolosis with pectus cartnatum which causes me loads of lowerback pain and neck pain felling like needles pricking on me when i carry some stuff.

I Going for neurology as there signs i have NF type 1 where it affects motor skills and sometimes i cant see objects which are there and other thjins

all together i have about 20 letters 4 xrays and loads of letters from doctors previous and new doctors

how will i go about to fill in the dla form

Will this be the first time you have applied for DLA?

If so, you need to think less about the disabilities you have and more about how they affect you from day to day. Do your disabilities affect you in such a way that you need extra help either getting about or looking after yourself? If so, then it is worth claiming DLA. If not then you are unlikely to qualify.

as above,you don`t get dla for illnesses/conditions but for care and or mobility issues arising from them,it sometimes helps to keep a detailed diary for 2 weeks of all care/mobility needs and go from there
