Help!!! Urgent - What have I done wrong - no court date given!!

Hi
Has anyone else not been given a court date but had the CCJ entered in their favour? I followed all the steps given - issued the notice of issue and got no reply from them in the time scale given, so rang the court to find out what I need to do next. They told me to get the judgement issued - which I have done, and it has been entered.
Yesterday I had another notice saving that because I got the name of my bank slightly wrong I need to fill in another form and send it back with £35 for name change. The top of this form - application notice - states I should provide this information for listing the application and how do I want the application to be heard??
a) at a hearing
b) telephone conference
c) without a hearing
Can anyone help me? How do I fill in this form??? Where have I gone wrong and will I still get my money??

Hi, i am in a similar situation to you regarding reclaiming my charges, all i will say is dont panic, i know it is easily said than done, but you must remember there is a 100% success rate with bank charge reclaims, if you have followed the procedures then you will be fine, pay the £35 for the mistake if you have to and ask for the hearing on the questionaire, the bank will either pay out before the date, or they wont turn up and you will be awarded the judgement in your favour.
