Account charge statement help please?

I just recieved my statements from my bank and i have tallied up the total I am owed. Do I need to send copies of these statements to my bank when I ask for my money back or can I just send a summary of all the charges I am claiming that I have typed up my self e.g
01/04/05 UNPAID D/D £32.00
01/09/05 O/DRAFT EXCESS FEE £30.00 .....etc.

I would rather just send a summary as I have written on the statements during my adding hehe (Stupid me)

Debt at highest April 07: Approx 10,000
currently: Approx £7000
DPA Request Sent off to Lloyds TSB on March 14th. This was settled in part shortly after. Asked for 1100, recieved 750 and settled.
