Template letters for reclaiming PPI

Please can some one direct me to where the template letters are for re-claiming PPI. Am trying to help my friend but don't know where to look as I never had any.

Hi there

The link posted by amersall will take you to a complaint/reclaim questionnaire, this is used instead of template letters now, complete and send to the ones responsible to setting up the finance with ppi, and keep copies for yourself as well.
