Mobile phone insurance - proof of purchase?

Hi,
I'm not sure where this goes but hopefully I can get some help here. I have damaged my phone screen so want to make a claim on my insurance (lloydstsb so lifestylegroupinsurance). However I lost the receipt that I got when I bought my phone. I asked three for some sort of proof of purchase and they have sent me this:

I’m sorry to hear that you’ve damaged your phone. Here are the details you need:
Phone reported damaged on: ****
The make and model of the phone you used: *****
IMEI (phone serial number): *****
Phone used from: *****
Current price of the phone: £00.00
We hope to see you back up and running soon. If we can help with anything else, please call us on 333 (free) from a Three phone or 0843 373 3333 (5p/minute from a BT landline, other networks may vary).

Is this all I need to make a claim? They have put the current price of my phone at £0, its a galaxy s2.. Whats that supposed to mean? Also they say used from 26th nov - 26th nov, which is just today. I've been using it for just over a year actually.
Would really appreciate some help as its driving me nuts

Yeh I think they must have, I got the phone for free but I still have 12 months of contract left to pay0
