Most insurers of mobile phones will use the same wording across all handsets. As iPhones are not repairable by anyone other than Apple, they will more than likely just go ahead and replace it. My insurance provider do not repair iPhones.
Also, with the refurbished handset: when your handset was lost/stolen/damaged, it was not new, therefore if you do not have new for old insurance, they are within their rights to provide a refurbished handset. Ask them about the warranty on it if anything goes wrong0 -

In addition, some will outsource repairs to authorised repair centre's - it doesn't need to be Apple directly who do the repairs.0
