Halifax credit card - PPI being charged even though i never took it out!

Hi, hoping someone can help.
My situation doesn't fall neatly enough into the usual categories so the template questionnaire isn't making sense for my claim.
Approx a year ago i noticed Halifax were charging me a one-off annual payment of approx £50 for ppi i never asked for. I rang them direct and they agreed to refund me for the few years they had taken it and also gave me £50 compensation (thereby admitting they were in the wrong?) I was happy with this, they dealt with is quickly and it seemed fair.
I've just noticed that they are continuing to charge me for it! Now i want to make a claim.
All the questions on the template are geared towards when did you TAKE OUT the policy - but i didn't, i was never asked and i never said yes. i did however make it perfectly clear i didn't want it and they've continued to charge me - so how do i handle this one?
Use the questionnaire but leave some questions blank and explain the situation at the end of the letter? OR ignore the questionnaire and just write to them with my compliant?

If it is an annual payment then it almost certainly isnt PPI. Possibly card protection?
Why not just write to them using good old fashioned words and sentences just as you have here? Far better than a template letter.I am an Independent Financial Adviser (IFA). Thanks for your response - they've sent me an Annual review document that describes it as 'Optional Payment Protection Insurance'
It actually details it as a monthly cost of £7.80 per £100 of monthly benefit.
I was presuming it was an annual cost simply because thats how they took the last lot of money from me but i might be wrong. Maybe this is something different.0 -

Credit card PPI is taken monthly and if there is an outstanding balance on the credit card each month.I am an Independent Financial Adviser (IFA).
