I'm afraid I think your letter is far too long - and risks confusing the issues. I'm not clear on what you actually want.
For example, do you really want an explanation of why various things happened? Or do you just want them fixed?
I'd be tempted to replace a large chunk of your text with a bullet pointed list of things you expect the LA to do.
 I absolutely agree with this.
I understand why you are annoyed. But you have a long list of complaints about things that have already happened and have been sorted.
As angry as you feel you need to sort out the serious matters that have yet to be addressed.
Get rid of the the things that have been fixed and list the things that are still outstanding.
Did you contact Shelter/CAB for help with this?0

I will just refer to one item in your letter...
 18. Following the refusal to supply the contact details for my Landlord, I checked with the Land Registry, and the details on my prescribed information and tenancy agreement do not match those held by the Land Registry. Can you please explain as obviously this is a serious breach of the Housing Act regarding the prescribed information being incorrect, and obviously a serious legal issue with the tenancy agreement being incorrect?
The address given on your Prescribed Information and the tenancy agreement is an address for the service of Notice, and this can legitimately be C/o the agent or another address and absolutely does not need to be the home address of the landlord.0

Agree but the OP is saying that they had the wrong name for the landlord!
OP I am now confused since you have replied as I posted.
Are you complaining about their shoddy service or do you want things put right? Or both?
I think you need to separate the different issues 0 0

 Both.
In order to move forward i first need to send letter of complaint to LA. After their response (hopefully I do get one), if I am not happy with it I can take action.
I am looking to take two completely different forms of action:
 1. Complaint to Property Ombudsman about breach of code of conduct (reason for complaints letter being so long)
 2. Deposit situation and wrong landlord details
 Am I making any sense?
Did you contact Shelter/CAB for help with this?
Am free for a few days now so will do all that, thanks.
To answer the question about serving notices my tenancy agreement states:
The Tenant is hereby notified that notices (including notices in proceedings) must be served on the Landlord by the Tenant at: Agency name, Agency address.
Thanks again for all your inputs! I will fix my complaints letter.0
