Post Office Fixed term deposit

I am thinking of placing some funds with the post office as their 2.95%, although not great, is about the best one-year fixed term rate out there right now.
Apparently, you need to go to the post office to apply. Ours is a small main post office, here in the Shetland Islands, so it could be a bit awkward to talk about our finances when others are behind me in a queue.
Is there a form you pick up in the post office to be sent off or should I take my passport, other identifications plus a cheque book with me and queue up?

Their web site says you can apply by phone, providing a freefone number.

HOWEVER, you can still apply if you can get an application form. They must receive the application form by next Friday, which is when the issue closes. Once the account is open, you have 7 days before the lid slams shut for 12 months (ie, no deposits or withdrawals allowed).0
