Limited capability for work questionnaire

Just I would like to know if the ESA50 the same limited capability for work questionnaire ?

    I don't understand the question.
    The ESA50 is the form usually sent out to the claimant of employment and support allowance.

    It is part of the evidence, along with that supplied by the claimant, the doctors report, the report of an in-person disability assessment, that is often used to determine the claimants limited capacity for work, or limited capacity for work related activity, which determines if the claimant is entitled to ESA, and what group they are placed in.

    Today we have received a form called ( limited capability for work questionnaire , ),my question was what that form ,it is the same for esa50?

    ESA50 should be printed on the front page.
    Have you applied for ESA, or are you on IS/IB, and have been informed you're converting?

    There is no ESA50 printed in front page ,
    Actually my husband is receiving Esa bane fit since 21/2/2012 till now he keeping send a seek notes ,they didn't tell us which group he is (I mean no support group or the other one) ,I don't know why ,It should be after 13 weeks when he applied for esa.
    And now I don't know for what this form that received today?

    if he is still supplying sick notes, he isnt actually on ESA, just in the assessment phase.

    has he had a medical?

    nona... it probably is the ESA50... here the document is in pdf form.. as above it should reference ESA50 at bottom and it should be version 03/11 (march 2011) http://www.direct.gov.uk/prod_consum_dg/groups/dg_digitalassets/@dg/@en/@money/documents/digitalasset/dg_195544.pdf

    The assessment phase is meant to last 13 weeks.. and for financial purposes it does.. but in practice it may take longer... many months longer for the WCA to begin! If this is the first ESA50 he's been sent and he isn't in the WRAG or support group then this is his first Work capability assessment beginning.
