Llyods TSB Credit Card - Question.

Hey all, I sent my claim off to Lloyds in May 2012 for repayment of their PPI. Lloyds let me know on 28th July 2012 they had received the letter telling me "I would hear from them shortly".
I hadn't heard anything by mid August so phoned them, and was told it was with the team, and they would contact me shortly.
August 28th I received another letter again telling me "I would hear from them shortly".
It's now the 5th of October and I am STILL to "hear from them shortly".
Question;
I have reclaimed PPI once before on a loan, it was dealt with within a month. I understand that with everyone reclaiming it will take a while, but nearly 5 months?
Is there a template letter available to complain to the Financial Ombudsman Service, I'm fed up of waiting now.

My sister in laws claim has been with the Ombudsman for 12 months so far, they don't rush !0
