HC1 Form Question

Does anyone know the current income threshold levels used to calculate via the HC1 form whether one is eligible for full (HC2) or partial (HC3) help with health costs?
I know it takes into account certain premiums etc, and found one relevant document through searching but not sure how out of date it is.
Thanks in advance for any help! Just trying to figure out what I may be eligible for as sending my form out this week. I've done this before and had varied decisions hence wondering what the current levels are.

NB I forgot to say that although the link says it's the rules for Scottish students it's the same for everyone (but cpag seems only to have got funding to provide free advice to Scottish students so it's the only free link I can find)
