I am surprised you needed a home visit with that list - perhaps your form was not specific enough and you GP did not provide enough information. I would say you would qualify for lower rate mobility at least.
I have had brain surgery - sorry if I am a little confused sometimes ;)

Just tell the EMP, in full, how your conditions affect your life. You can have a friend with you, if you wish.

If the EMP asks you to sign anything, then check what he has written first.

You can tell a lot about a woman by her hands..........for instance, if they are placed around your throat, she's probably slightly upset. ;)

Personally i would think you would get high rate mobility and low rate care. Just going from experience of a family member in a similar situation. Although with all your problems i think you should get high rate care as well.
As you most probably have read D.LA isn't an easy benefit to get.
When the doctor comes to your house he will basically go through all that you have already written on the form.Asking you the same questions. Just be as honest and tell him what life is like for you on your worst day. I was their when my family member had a home visit. The doctor was a really kind man. Just went through everything with us and then said we would hear within a few weeks. My family member was pleased with the result.
    Hope this helps
    Good luck.
