PPI claim - advice please

I would like to claim for PPI mis-selling. I have copied a template letter but I notice it doesn't suggest I put in details like the dates, the premiums I paid, nor the address I had at the time.
Can anyone tell me if all this info is required? The policy I'm trying to claim mis-selling for dates back to the late 1980s/early 1990s and I don't have all the details any more.
I suppose I just want advice from someone who has been through this - do the companies usually come back and ask for all this info if you don't provide it in the first letter, or will they just check their own records?

That's what I thought - still there's no harm in trying.
I just wondered how much info they will need. Maybe it's best if I just do a longer letter and give them all the dates and premiums I can find details for.0 -
