Returning to work after sickness

I have heard that going back to work after being on incapacity benefit means you can get some extra help in the form of a grant of £100 and payment of £40 per week if you earn under £288 per week and work over 16 hours.
Do you have to apply for this or is it automatic when you let the jobcentre know you are ending the incapacity claim because you are working ?
Also what if you take a position with no contracted hours, but you do actually work a lot such as bank staff? Does that count?

the £100 is supposed to be automatic, but local JC+ lady sent the forms to me just in case,
just as well as Tuesday was my last esa payment that got screwed up by them, and i didn't have the extra money.
so will have to sit and wait for a week or two before i ring to see what went wrong this time.0 -
