Carpet fitting advice

Hi
I had carpet fitted in our bedroom and on the stairs and landing at the weekend.
I am looking for any advice that you have regarding the fitting of the carpet at the threshold between the landing and the bedroom. We currently have a large dip from each room to the door bar. The dip is so large (around 2cm at the deepest point) that you can see the carpet on the bedroom side of the door when the door is closed and you are standing on the landing side of the door. Just to confirm, the door is not too short (it is actually a little too close to the carpet when fully open).
I have never noticed such large "ditches" between rooms in other people's houses. Is there something that the fitter could/should have done better to avoid this issue?
Any help would be greatly appreciated.

This is the same at all three door thresholds off of the landing. The floor was perfectly even before he started! May sound like a stupid question, but should the underlay and carpet go all of the way up to the centre of the door bars?0

The underlay and carpet from different rooms should be butted together, then the threshold strip screwed on to hold the underlay and carpet in place.
Take a threshold strip off and see if there's a gap.0 -

How thick is your carpet and underlay?
If, like mine, they are thicker than average, even if they abut right up to the doorbar, there will be a dip. The only way to avoid such a dip would be to pack under the doorbar, as krisdorey says.
Personally, though I wouldn't favour that option - I'd rather have the dip either side of the doorbar.0 -

The previous carpet and underlay worked much better with the current doors sizes.
The new carpet is slight thicker.
I understand that the door might need to be slight adjusted at the bottom due to thick underlay and carpet.
I but assumed when the carpet fitter accessed the job prior to carrying out his work, would of spotted this as an issue, or at least when on the job and should of advised me. Do you think that this is a fair assumption?0 -

That is a fairly sizeable dip (krisdorey's link also works for me).
