Atos form

I got the dreaded ATOS questionnaire form last week and I need to return it back by 2nd October.

I have a heart condition, under active thyroid, polysistic ovaries, and I suffer from depression.

I've got letters going back to 2001 when my heart condition was diagnosed after a 24 hour ecg showed I suffered from sinus tachycardia, which a further test in 2010 showed it was still present.

I find going up the stairs, walking and doing everyday tasks a struggle as I get out of breath so easily.

Should I put this letter in with the questionnaire to help my claim!?

  • You should only include evidence that relates to your current condition. The evidence needs to relate to your ability to work, not simply state that you have x condition (especially one that can be asymptomatic).
  • Ideally evidence would be recent.. doesn't sound like it could do any harm though. But focus on those descriptors.. they're all that matters... and evidence that doesn't inform in relation to them in the present will be of limited use.

  • Had a laugh in the post this morning from DWP; they sent me a letter saying they tried to phone me about the benefit switch over and to discuss the questionnaire; I've already been contacted and spoken to and am sending the questionnaire back next week; I dont know why the harassment and surely they should have put a note on their system I have already been contacted!!

    Oh get ready for a right royal run around... I keep promising myself that DWP ESA admin stories will no longer surprise me and I keep letting myself down. I understand that the case management system is detested by staff as it is so poorly designed and outdated.. it seems to send out irrelevant letters or wrong letters and so I understand staff sometimes turn features off... this then seems to result in important letters not getting sent. The DWP has never failed to send a letter to me regarding £25 cold weather payments due to me receiving IR ESA.. yet it has never managed to send me a (correct!) decision letter on entitlement to ESA which is what I depend on to live.

    Seeing the CAB tomorrow about helping me fill the ESA form in, so I can send it back this week; the sooner the better!!

    I've part done the answers on a separate sheet of paper so they can see if what I wrote is relevant; I just find the form confusing, and I am not good at explaining things.

    My GP says she can provide a medical report from her end if they need to contact her direct; I can still put a copy of the letter from the hospital going back to 2001 showing the tachycardia being picked up.

    I had another ecg done in December 2010 which still picked my heart problem up; so no change there!!

    If I get called up for a medical and I fail which I am expected to judging by peeps on this site have been already, I shall appeal.

    I just cant understand why the Government is making these changes all the time; its not fair and downright stressful!!
