FACE Information & Assessment form

Does anybody know what a FACE v5 assessment is for?

My psychiatrist has arranged for this form to be filled in by my carer over the weekend, but I have no idea what the reason is.

They want it back by Monday.

It's 5 pages long and I don't want them to do it if it means that I have to have more people poking their noses into my life.

Thanks in which case they are attempting to infiltrate my life a bit more.

i presume I have the right to refuse to co-operate, I just wonder what they would do if I did.

A member of my family said years ago that it is best if you keep well out of the way of the doctors and social services, because once they have hold of you you stand to lose your independence.

Wise comments indeed. I've had nothing but agro since I asked for help.

If you want to claim money from the public purse then you have to accept a greater level of involvement from public services.

I never said anything about claiming any money.

I am only concerned that my life now seems to revolve around hospitals and social services.

I just wanted to know what this was about, and as I thought, it would seem that social services want to have more of a say in how I live my life.

My only worry is I don't know what their powers are if I refuse to co-operate with them.
We all hear of how they can be unreasonable and like nothing better than to take over peoples lives.

I have spent the best part of the last 18 years keeping under the radar of both them and the hospitals. Managed it for a while, in fact 11 years! Now the !!!!!!s have got hold of me again.

But you are claiming benefits so you do need to be co operative.
