Now, to answer your supplementary question...
It could possibly be because the applicant could not provide consistent information about their name.
I can see that leading to doubts about all sorts of things.
But no-one except the bank will be able to answer that question, will they?
It was accepted both times on the website, the first time the required documentation was sent, then a month later she called to chase as had heard nothing, she was told it was because of no middle name on the app.
So she reapplied,providing middle name, it was accepted, thus awaiting forms to resend docs and one day later an email stating declined with no reason.
This to me seems based on discrimination, what else could it be?0 -

Well, I honestly doubt your partner is the only black woman in the UK to have an account with this bank, so discrimination on grounds or race and gender seem somewhat unlikely.
Nobody is obliged to do business with anyone else - the bank don't want your business for whatever their reasons are, so try another - no point getting hung up about it when it'll go nowhere. There are plenty of banks out there. If one of them offers her an account, all done. If none of them will, then there's clearly some other problem with her status that makes them wary. Banks are in the business of bank accounts, so they don't decline people just for fun.
(If it is any consolation, Coutt's (who the Queen banks with) is just dumping clients who they don't feel are suitable for their products - anyone earning under £100k seems to be in the firing line - this is just businesses choosing their customers as it is in your case too)0 -

Of course the box had no * by it! This doesn't mean it is optional, it means fill it in if you have a middle name, leave it blank if you don't!
It's not discrimination though, like someone has said, it can be difficult for foreign nationals to get bank accounts here, and someone who isn't sure of their own name and has made multiple applications with (albeit slightly) different names would probably raise a red flag somewhere.
