DLA - What to do when you can't even cope with filling out the form?

For the first time in my life I have had to stop working. Over the past few years a period of depression has become so bad I have ended up having a full scale breakdown. I have had counselling and am about to start some more with MIND, am on high-dose medication and under the close supervision of my GP.
I was advised to apply for DLA. I began filling in the form but found it so hard to concentrate and deal with, I still haven't done it. So far it hass sat there half finished for 3 months.
Is there anywhere that could help me? I feel so frustrated.

Have you tried CAB? (Google for your nearest one)
Some CAB have outreach advisors - it might tell you on your local website if they do or ask if you telephone.0 -
An advocate from MIND helped me when I claimed DLA. As you are having counselling with them, they may be able to refer you to someone who can help.0

One option is to send in the partially completed form, with a note that you can't manage it, and to contact your doctor for more details.
Include anything you think might be relevant that you can face putting down.0 -
