Aqua repaying in full cash?

How does it work, I don't really understand.
So, for example, if I for some reason use an Aqua credit card to withdraw cash abroad (€100), I don't get any load, but I get a charge for taking out cash (£3), and then interest even if repaid in full?
How does the interest work? How is it paid? Can I pay it all off at a once?
So if the APR is 40%, how will the €100 look if repaid in full? And what does it mean to repay in full — right away, or at the end of the billing month?
Sorry for my ignorance, I'm never in credit (and don't plan to be, but just so I know how bad the penalty is for using Aqua for cash).
Update: I may still get an Aqua card, but just got a Metro Bank debit card, so now I can take out cash in a foreign currency when abroad without any additional fees apart from the MasterCard exchange rate!
