Vanguard hold their investments via a 3rd party with regular auditing.I am not a financial adviser and neither do I play one on television. I might occasionally give bad advice but at least it's free.

Virtually Nil. These are serious major companies with £billions under management. As far as I know it has never happened. If it did happen, it would be by a rogue employee and the company would be responsible for losses.
Unfortunately it has happened - any queries please address them to Bernie Madoff, Federal Correctional Complex, Butner, North Carolina. Although don't get your hopes up as he won't be available to talk to you until 14th November, 2139.
Seriously though, the control environment in the asset management industry, the vast reams of legislation around client funds/client assets, and the enormous regulatory oversight (particularly on US firms such as Invesco, Vanguard, etc), means this should feature somewhere between "alien invasion" and "Mr Blobby's comeback tour" on your list of things to worry about.0 -

We were once on holiday with toddler-sized daughter and she was being fractious in the extreme.
We decided to take her to a Crinkley Bottom park nearby so she could enjoy the attractions, wife could enjoy the gardens, daughter would sleep well at night for once, and we'd get blissfully hammered.
She hated it and screamed throughout. So did we.
Mr Blobby's comeback tour will now keep me awake at night.
