ESA medical certificate

Hi

I started my claim for ESA last week, they requested my birth certificate and a medical certificate which i was told i would need to go to the gp, i went today and he's give me a sick note, is this what they mean by a medical certificate? Does this mean i have to keep getting sick notes to send to dwp or is this just for the 13 phase before a decision is made on what group i will be put in to.

    Not sure why Birth certificate has been requested... might be something to do with proving identity or age.

    But medical certificate.. yes sicknote is the same thing.. today the document is called a fit note.. refer http://www.dwp.gov.uk/fitnote/

    You'll need to keep supplying continuous sicknotes until you pass the Work Capability Assessment. That assessment is scheduled to take place around the end of your assessment phase of 13 weeks but in reality it could vary. If you pass the WCA then you'll get placed in WRAG or Support Group and then won't be required to supply any more of these fit notes. If you fail the WCA and decide to appeal then you may well need these fit notes to continue to be supplied to get assessment rate ESA while you wait for that appeal to happen.

    thankyou for clarifying that, hes gave me one for 4 weeks so after that period is over i just get another and continue to send to the free post number.

    I wasn't sure why they wanted the birth certificate either but ive sent it as request.

    No worries. If it is any help, when your time comes for assessment then you'll almost certainly be sent an ESA50 form to complete. There is a pdf version you can take a copy of and complete electronically.. although ultimately it need to be printed and signed and posted (along with any supporting evidence you have) come the time if you decide to use it. But if you get bored it might be worth your while looking at it or completing some of it. If you do complete it I'd strongly advise having at your side the descriptors for the WCA.. the descriptors being the statements of disability that are tested for in the WCA and determine entitlement.
