Which forms are sent to medical professionals when ATOS "healthcare" are requesting information? I know one is ESA113. Are there any other known request for information forms?

That is the only one I know of.0

That's right, the ESA113 goes to your declared GP. However, you can send any supporting medical evidence in. As a rider, DWP will not request evidence from your GP or any other HCP.0
