The survey should only take about 10-15 minutes to complete and, as a thank you, all those who complete the survey in full will be entered into a prize draw with the chance to win a meal for four at Jamie's Italian.

This prize draw is organised by the Jamie’s Italian whose address is 2 Gay Street, Bath BA1 2PH.
The prize draw runs from Tuesday 17th July 2012 until 23.59pm (GMT) on Tuesday 31st July 2012.
The closing date for this prize draw is 23.59pm (GMT) on Tuesday 31st July 2012.
The winner will be selected at random and contacted via email no later than midday Tuesday 7th August 2012. The email will be sent to the email address which the winner has provided in the survey.
If the winner cannot be contacted or is not able to accept the prize within 2 months of the prize draw's closing date, then we reserve the right to award the prize to another participant.
The prize is a £150 voucher to spend on food, drink and service charge for up to 4 people at Jamie’s Italian. The prize is subject to availability and must be redeemed by Thursday 7th February 2013.
This offer cannot be used in conjunction with any other prize offer.
The competition is open to all residents in the UK over the age of 18 but is closed to any employees (and members of their families and households) of Jamie Oliver Enterprises Limited, its associated companies and of any co-promoters, competition sponsors, prize providers and any of their associated companies.
