Return to work after ICB

Hi
I am hoping to be offered a job after 3 years of Incapacity Benefit. I have a long term condition which will never get better but I am determined to go back as I am bored out of my brain and tired of being on benefits. I am aware of the rule that if I become too ill again I can go back to the benefit but pray that I will be ok as I have been doing voluntary work for a while now.
I have told the employer I have the condition on my application but I have not mentioned that I claimed ICB. My husband only works part time too as we have an adopted child with complex special needs (I can't lift him etc) so my ICB helped with our life costs.
My question is when I start work a) what do I give them? (I have no P45 as far as I am aware) b) Will they need to know?
I do not want them to think badly of me or that I will go off sick immediately.
